How to Subscribe

To be a Base Telecom long distance subscriber fill in the following Application Form:

Your company name (if this is a business account), your first & last name, address, phone and fax numbers.

If you choose prepaid service in “Payment/billing information”: you have several options:

1. If you’d like to prepay by credit card, fill in your credit card information (number, expiry date, legal name of cardholder), amount and date. Mark appropriate box and sign below.
   
   
2. If you’d like to prepay by check send your check with Application Form

If you choose postpaid service in “ Payment/billing information” you have the following options:

1. If you provide your Credit Card for security purposes fill in your credit card information (number, exp. date, legal name of cardholder). Mark box “…Security purposes” and sign.
   
   
2. If you provide your SIN for security purposes, enter you SIN in Credit Application.
